Health insurance renewal in Italy

Written from published rules on 26 August 2026. Not monitored for changes since. A starting point, not official advice — confirm anything that matters with Agenzia delle Entrate (Italian Revenue Agency), in collaboration with the Ministero della Salute (Ministry of Health) and local Aziende Sanitarie Locali (ASL - Local Health Authorities).

Renewing the Italian Health Insurance Card (Tessera Sanitaria) typically involves receiving a new card automatically when the previous one expires, provided one is registered with the Italian National Health Service (SSN) and has a valid tax code (Codice Fiscale). In certain cases, particularly for those not automatically receiving it or requiring a replacement, an application might be necessary.

Your timing

Most individuals receive a new Health Insurance Card automatically at their registered address a few months before the expiration date of their current card. It is generally advisable to confirm receipt or inquire about non-delivery approximately 1-2 months before the current card expires, to allow time for reissuance if needed, especially considering potential postal delays or administrative processing times.

Start 60 days ahead

This lead time accounts for the typical automatic mailing process, which usually occurs a few months before expiration, and provides a buffer for inquiring about non-receipt or applying for a replacement if the automatic delivery fails.

The process

  1. 1Verify the expiration date on the current Tessera Sanitaria.
  2. 2Ensure the registered residential address with the Italian Revenue Agency (Agenzia delle Entrate) is current, as the new card is typically mailed automatically to this address.
  3. 3If the new card is not received automatically several weeks before the old one expires, contact the Agenzia delle Entrate or the local ASL for assistance.
  4. 4For individuals who do not receive the card automatically or need a duplicate, apply online via the Agenzia delle Entrate website or in person at an Agenzia delle Entrate office or ASL counter.
  5. 5Await the delivery of the new Tessera Sanitaria to the registered address.

What to bring

  • Valid identity document (e.g., identity card, passport)
  • Codice Fiscale (Italian tax code)
  • Previous Tessera Sanitaria (if available, for reference)
  • Certificate of residence (if applying in person and current address needs to be verified or updated)

Who handles it

The issuance of the Italian Health Insurance Card (Tessera Sanitaria) is generally free of charge.

What commonly goes wrong

  • An incorrect or outdated registered address can lead to non-delivery of the new card.
  • Ensure active registration with the Servizio Sanitario Nazionale (SSN - National Health Service) through the local ASL; continuous registration is a prerequisite for receiving the card.
  • If applying online, ensure access to the specific services provided by the Agenzia delle Entrate (e.g., through SPID, CIE, or CNS).
